Three jobs, not one

Search is splitting into two habits. Patients still google, and a growing number of them open ChatGPT or Perplexity and ask which surgeon to trust. Those two habits reward different work, so the agents do all three jobs every week rather than optimizing for the search box alone.

This is the actual loop, in the order it runs. It is written down because the agents follow a written playbook, and a practice paying for the work is entitled to read the same document.

    Orient, and check last week first

    Every run opens by re-reading the last two runs and re-checking what they shipped against your live site. A change that quietly regressed outranks anything new, so it gets fixed before the run starts fresh work. This one habit is why the improvements accumulate instead of cycling.

    Measure against last week

    The agents pull your Search Console data and diff it against the stored baseline: the pages gaining, the pages decaying, the pages whose impressions climbed while clicks stayed flat, and everything sitting in striking distance at positions six through fifteen. That diff is written down, and then the baseline is replaced, so next week has something honest to compare against.

    Mine your own property

    Rotating focus across runs so nothing goes permanently unexamined: indexation counts against what is live, orphaned pages nobody links to, redirect chains and where they actually land, pages losing ground that need a refresh, and the internal link graph from hub to cluster. Most of the ranking left on the table in this category is sitting inside a site that was launched once and never tended.

    Map what your competitors publish

    The agents pull competing practices' sitemaps, build an inventory of every page they publish, cluster it into topics rather than keywords, and find the gaps. What everyone covers is table stakes.     Structure passes for AEO and GEO: direct answers, definition blocks, comparison tables, headings that match real questions, schema that states only what the page genuinely shows, and an AI-facing page that spells out the plain facts about your practice. The agents also record which sources the assistants cite for your money questions, which is the only reliable way to see whether the work is landing.

    Research what changed this week

    Search is moving faster than any playbook written a year ago. Every run spends time reading what practitioners are publishing about SEO, AEO, and GEO right now, then logs each tactic with a verdict: adopt it, test it, or reject it and why. The log is shared across every property we run, so a tactic is only ever evaluated once.

    Ship a small number of verified changes

    A run ships at most three improvements, and each one is built, tested, and confirmed live before the next one begins. Two solid changes beat three where one is shaky, and the agents are instructed to stop early rather than pad the run. Every change is a real commit with a real diff, so there is always an audit trail.

    Prove it, then write down what was learned

    Nothing counts as shipped until the live page has been fetched and the exact change confirmed in it, cache bypassed. Then the run appends what it did and what it learned to a permanent log. They never invent a fact

Every number, claim, and comparison that reaches one of your pages has to trace to your own data or to a source cited on the page itself. If a tactic needs a claim we cannot source, the tactic waits.

They never send anything on your behalf

No emails, no direct messages, no posts, no review requests, no outreach of any kind. The agents work on your properties. Talking to patients stays entirely with your practice.

They never mark their own homework

The agent that writes a change is never the one that approves it. A separate reviewer examines the diff before anything merges, and a separate verification step confirms it on the live site afterward.
